How it works

Benzoyl peroxide works through a straightforward but powerful mechanism: it releases oxygen radicals within the pore that are directly lethal to Cutibacterium acnes, the bacteria implicated in inflammatory acne. Unlike topical or oral antibiotics, bacteria cannot develop resistance to this oxidative mechanism, which is why dermatologists still reach for it first-line decades after its introduction and why it remains a standard partner ingredient in prescription acne regimens. It also has mild comedolytic and anti-inflammatory effects, helping unclog pores while calming existing lesions.

The evidence

Benzoyl peroxide is one of the most extensively studied acne treatments in dermatology, with decades of randomized controlled trials confirming its efficacy for inflammatory acne, both used alone and combined with topical antibiotics or retinoids specifically to prevent bacterial resistance from developing against those other agents.

Suitability

Best suited to oily, acne-prone skin with inflammatory lesions — papules and pustules respond faster to benzoyl peroxide than purely comedonal, non-inflamed acne. Dry or sensitive skin should start low and build tolerance slowly, as peeling, dryness, and redness are common in the first couple of weeks.

Concentration

2.5% is as effective as 5% or 10% for most people, with substantially less irritation, so I rarely recommend going higher unless directed by a dermatologist for a specific reason.

Conflicts & combinations

This is the ingredient I flag most often for conflicts: it oxidizes and deactivates both retinol and retinal on direct contact, and it can also degrade vitamin C's active form. Use it at a different time of day or on alternating nights from those actives. It pairs well with salicylic acid and niacinamide in a well-sequenced routine.
